BİLİMSEL YAYINLARDA SAHTEKARLIKLAR: TÜRKİYEDE KAÇ SAHTEKAR VAR???
İlaçlarla ilgili yayınlar ne kadar doğrudur? İlaç firmaları negatif sonuçları, ilerine gelmeyenleri, yan etkileri hatta ölümleri saklıyor mu? Dergiler negatif sonuçları yayınlıyor mu? Araştırmacılar negatif sonuçlarını yayınlıyor mu? Bu sorulara evet demek mümkün değil. Maalesef bir çok klinik çalışma yayınlanmadan sümen altı ediliyor, otoriteler, sağlık personeli, hastalar ve kamuoyu yanıltılıyor ve kandırılıyor.
Birçok araştırmacı yayınlanmamış araştırmaların alarm verici olduğunu, bunlardan elde edilen sonuçların sistematik olarak yayınlananlardan farklı olduğunu ileri sürüyor. Bazı araştırmalar sahtekarlıkların ortaya çıkmasını önlemek için yayınlanmıyor. ABD de bilgi edinme kanunu çerçevesinde FDA dosyalarını inceleyen araşturmacılar FDA’in klinik araştırmaların %11 inde ciddi aksaklıklar (çok kötü aratırma tasarımından sahte sonuçlara kadar) olduğunu ortaya çıkarttılar. ‘British Pharmaceutical Industry’ medikal direktörü Dr. Frank Wells ‘Kesinlikle bu problem var ve British klinik araştırmacılar tarafından yapılan araştırmalarda anlamlı miktarda sahtekarlık mevcut’.
Dr. Sheng Wang (asistan profesör) Boston Üniversitesi Tıp Merkezi Kanser Araştırma Merkezinde çalışıyordu ve 2011 Temmuzunda işine son verildi. Sebep? Sahte makale yazmak! Wang’ın sahtekarlığı ‘Office of Research Integrity of the US Department of Health and Human Services’ tarafından ortaya çıkartıldı ve Wang, National Cancer Institute tarafından desteklenen ve çok ciddi dergilerde (Oncogene and Molecular Endocrinology ve Molecular Endokrinology) yayınlamış olduğu, tümör büyümesinde genlerin rolü konulu iki sahte makaleyi geri çekti. İşin garibi Kanser Araştırma Merkezi Direktörü Dr. Douglas V Faller de bu makalelerin yazarlarındandı!..
Bu bilim dünyasında ne ilk vede ne son sahtekarlık olacaktı. 2001 Yılında 22, 2006 da 139, 2011 de 339 makale bilimsel dergilerden geri çekildi. Merak edenler geri çekilen makalelerin bir kısmını www.pubmed.gov da (anahtar kelime: retracted publications), bir kısmını Thomson Reuters Web of Science (11600 dergiyi içermekte) bulabilirler.
Dünyanın önde gelen tıp merkezlerinden MAYO klinikte senelerdir yapılan kanser araştırmaları bilimsel sahtekarlık yüzünden çöpe gitti. Dokuz prestijli dergide yayınlanan 17 araştırma geri çekildi. Bu çalışmaların hepsi immün sistemin kanserdeki rolü ile ilgiliydi ve hepsi masa başında yazılmış uydurma makalelerdi… 18 Dergi Alman anestezist Joachim Boldt’un isminin olduğu 89 makaleyi geri çekme kararı aldı. Daha önceki yazılarımızda uzun boylu anlattığımız Baystate medical Center’den anesteziyolog Scott Reuben’in ‘multimodal analjezi’ ile yazdığı ve ciddi dergilerde yayınlanan 21 makalenin tam bir sahtekarlık ürünü oldu ortaya çıktı.
İndirilecekler bölümünde yayınladığımız ‘Corporate Crime’ın yazarı Dr. Bratithwaite ‘FABRİKASYON’ MAKALELERİN çok yaygın olduğunu, ilaç firmalarının sağlık otoritelerini nasıl kandırdığını detaylı bir şekilde açıklamakta. FDA başkanlarından Dr. Ley daha 1969 larda ABD Senato soruşturmasında bir yardımcı doçentin 9 firmaya ait 28 ilacın araştırmasını yaptıüını ve ölümlerin saklandığının ve olur formlarının hastalar öldükten sonra imzalandığını açıklayarak problemin büyüklüğüne dikkat çekmekte Enbüyük skandallardan birisi ise 28 ilaç firmasının 82 ilacının klinik araştırmalarını yapan bir CRO nun (Clinical Research Organisation) ölen, hastaneden çıkan ve araştırmayı terkeden hastaların yerine yeni hasta koyarak sonuöları firmalar lehine çarpıtması ((US Senate, Competitive Problems in Pharmaceutical Industry, 1969, pts6,7,10). Bu firmaların denetçileri acaba ne yapıyordu? FDA, 1977-1980 arasında araştırma sonuçlarını çarpıtan, manipüle eden, yalan rapor yazan 62 doktor tesbit etti (Braithwaite, Corporate Crime, sayfa 53, 1984). FDA ‘Division of Drug Experience’ direktörü Judith Jones, ilaç firmaları bir klinikte negatif sonuç aldıklarında araştırmayı pozitif sonuç verecek kliniklere taşıdığını, negatif sonuçların çok nadir yayınlandığını, ilaç firmalarının kendilerine sempatik (!) araştırmacı bulmakta zorluk çekmediklerini açıklamakta (Cured to Death, Melville A ve Johnson C, Angus and Roberthson Pub., 1982). ABD de klinik araştırmalarda hasta başına doktora ödenen meblağın 500-25.000 dolar olduğu düşünülürse işin boyutu daha iyi anlaşılır. Bazı doktorların ilaç firmalarından aldıkları para maaşlarının 2-4 misli daha fazla olabilmekte. Türkiyede genellikle doktorlar hasta başına birkaç yüz-birkaç bin lira almakta. Bazı kliniklerde ise hasta başına 15.000 TL alan doktorlarımız mevcut, sahte araştırmalar ve sonuç uydurmalar da ABD den farklı değil.
Bilim dünyasındaki en ciddi aldatmacalardan birisi hiç şüphesizki SEROQUEL olayıdır. Bu konuda daha önce bu sayfalarda onlarca yazı çıktı (bknz http://www.klinikfarmakoloji.com/index.php?q=node/463 , http://www.klinikfarmakoloji.com/index.php?q=node/463 ). Daha önce yayınladığımız Seroquel dosyasında şirket içi gizli haberleşmeleri, bilgileri nasıl sakladıklarını, makaleleri nasıl manipüle ettiklerini bütün açıklığı ile göreceksiniz. İsterseniz Google’a şöyle bir soru sorun!! ‘Astrazeneca burried data’.. 3.550.00 den fazla web sayfası bulacaksınız. Kısaca özetlersek: AstraZeneca işine gelmeyen (diyabet yapıcı etkisi , kardiyak ölümler, QT uzaması ve diğer ciddi yan etkiler) en az üç klinik çalışmayı saklayarak, bunların içinden işine yarayanları seçip (cherry picking) ilaç lehine yeni bir makale yaratmış ve bütün bunlar mahkeme kararı ile el konulan firma içi gizli yazışmalarda (1999) ortaya çıkmış.Firma yetkilisi Tumas 15, 31 ve 56 çalışmaların gömüldüğünü AstraZeneca yetkililerine bildiriyor… Neyseki bu Türkiyede olduğu gibi yapanın yanına kalmadı. AstraZeneca hakkında binlerce tazminat davası açıldı ve AZ endikasyon dışı satışları için 520 milyon dolar ceza ödemeye mahkum oldu. Bu ceza firma için devede kulak misali, Seroquel’in 2008 satışı 4.45 milyar dolar!
Mahkeme kararı ile açılan AstraZeneca dosyalarında Seroquel’in Risperdal ve haloperidol ile kıyaslandığında ikisinden de daha kötü olduğu, davranış, anksiyete, hostilite yönünden plasebodan biraz üstün ama diğer ilaçlardan daha az etkili olduğu ortaya çıkıyor. Gizli belgelerde seroquelin dünya emniyet sorumlusu Wayne Geller’in daha 2000 yılında ilacın diyabet yaptığını bildiğini ve bunu firma yetkilileri ile paylaştığını gösteriyor. Buna rağmenAğustos 2000 de firma FDA’e verdiğicevapta ahlaksızca yan etkiyi saklıyor ve ‘Seroquel’in insanda diyabet yapacağına ait hiçbir data yoktur’ diyor. Ancak Eylül 2003 te FDA Risperdal, Seroquel ve diğer yeni antipsikotiklerin diyabet riskini artıracağını duyuruyor. Daha sonra 2.4 milyar dolar ceza ödüyor.
Federal soruşturmacılar Kentucky Üniversitesi eski kıdemli biomedikal araştırma sorumlusunu 10 yıldır bilimsel sahtekarlık yaptığı gerekçesi ile suçlu buldular. Dr. Eric Smart grant müracaatlarında sahte bilgi vermek, sahte gelişme raporları hazırlamak, sahte makale yazmaktan suçlu bulundu. Kardiyovasküler hastalıklar ve diyabetin moleküler mekanizmalarını araştıran Dr. Smart, 45 sahte grafik, 7 sahte grant müracaatı, 3 gelişme raporu ve 10 sahte makale yayınlamış. Hiç deney yapmadığı halde ‘knockout’ farelerde yaptığı araştırmalar da olmuş ve makalelerine yüzlerce defa atıfta bulunulmuş. Ahlaksızlığı ortaya çıkınca yayınladığı 10 makale düzeltilmiş veya geri çekilmiş. Smart bu sahte araştırmaları için Ulusal Araştırma Enstitüsünden (NIH) s milyon dolar destek almış ve 10 sene kimse farkına varmamış! Foyası meydana çıktıktan sonra Kentucky Üniversitesinden 2011 de istifa eden Smart halen Kentucky de bir okulda kimya öğretmeni olarak çalışmakta. Herhalde bilimsel sahtekarlık nasıl yapılır kursu açmıştır!!!
Sahte makaleleri geri çekilme rekoru Japon ansteziyolog Yoshitaka Fujii’ye ait. Fujii 1992-2012 arası tam tamına 172 sahte makale yayınlamış ve kendisi bu sürede Tokyo Medical and Dental University, University of Tsukuba ve University of Toho’da çalışmış. Fujii bu dönem içinde yayınladığı hastaları da hiç görmemiş.
Massachusetts halk Sağlığı laboratuvarında çalışan adli tıpçı kimyager Annie Dookhan kabaca 34.000 adli ilaç vakasında analiz yapmadan rapor yazarak binlerce kişinin yanlış yere suçlu veya suçsuz bulunmasına sebep olmuş. Sahtekarlığı, herkesin senede 3000 analiz yapabildiği bir laboratuvarda bunun 9000 analaiz (!) yapması sonucu ortaya çıkmış.
Prozac, Paxil gibi ilaçları üretenler araştırmaların üçte birinden fazlasını hiçbir zaman yayınlamazlar. Yayınlanmış araştırmalarda antidepressan kullananların %60 ının anlamlı iyileşme gösterdiği, %40 ının ise plasebodan farksız cevap gösterdiği belirtilir. Bu rakamlara firmaların gizlediği, yayınlamadığı çalışmalar ilave edilirse antidepressanların belirli hasta gruplarında çok az etkili olduğu ortaya çıkar (N Engl J Med 2008; 358:252-260January 17, 2008, New York Times 17 Ocak 2008).
Rath Vakfına göre (bakınız sektörden sayfası) farmasötik endüstri bir ‘sağlık endüstrisi’ değildir. Farmasötik endüstri varlığını hastalıkların devamı ve yayılmasından alan bir ‘yatırım işidir’. Senelik 500 milyar dolardan fazla kar hastalıkların devamı sayesindedir. Maalesef son 15-20 senede ilaç sktörü tarafından yapılan işler bu iddiaları desteklemektedir. University of California San Francisco Tıp merkezi araştırmacıları yaptıkları analizde statinlerle ilgili 200 kafa kafaya yapılmış araştırmada firma tarafından desteklenen çalışmalarda sonuçlar sponsor firma lehine 20 misli daha fazla ( http://www.sfgate.com/cgi-bin/article.cgi?f=/c/a/2007/06/05/DRUGS.TMP). Türkiyede durum nedir? Hiç şüphesiz ki bütün dünyada olan bu olaylar Türkiyede de olmaktadır. Klinik çalışmalarda denetim kağıt üzerindedir. Bazı araştırmacılar efendilerini memnun etmek için negatif sonuçları pozitif yapmaktadır. Bazıları ise yeteri kadar hasta bulamadıkları için uydurma hasta üretmektedirler. Seneler önce yaptığımız çok merkezli bir migren araştırmasında bir kliniğin tüm sonuçlarını sahte olduğunu anladığımız için o kliniği projeden atma zorunda kalmıştık. Başka bir araştırmada ise sonuçların tutarsız olmasını gördüğümüzde formları dolduran uzman doktora neden uydurduklarını sorduğumuzda açık yüreklilikle ‘parayı hoca alıyor, angaryayı biz çekiyoruz’ demiştir. Başka bir araştırmada ise ön şart olarak hastaların daha önce sumatriptan kullanması gerekiyordu. O dönemde o projeye yetecek sc sumatriptan kullanan bulunamadığı için meşhur bir nörolog Türk usulü basit bir sahtekarlık yaparak, tüm hastalara önce sumatriptan vermeye başladı ve diğer triptan firması bunu bildiği halde bizi ilgilendirmez, biz sonuca bakarız diyebildi… Bir klinikte 30 hasta dosyasının 2 saatte doldurulduğunun şahitleri mevcut… Bir gün çalışkan bir asistan bana gelerek hocasının negatif sonucu kabul etmediğini, pozitif çıkana kadar denyi tekrarlamasını istediğini söyledi ve sonunda deneyler pozitif çıktı(!!!)… Sahtekarlığın diğer bir yöntemi dataları yağlamak veya negatif datayı istatistiğe sokmamaktır. Bunun örnekleri hem temel tıbda hemde klinik araştırmalarda oldukça fazladır. Meşhur bir hocamız hesaplarını (istatistiklerini) hep akşam herkes gittikten sonra yapar ve sabah istenilen sonuçla makalesini yazardı!!!!… Türkiyede zakkum olayına kadar insan üzerinde araştırma yapmayı düzenleyen hiçbir yönetmelik yoktu. Zakkum olayından sonra ısrarlarımızla toplanan 30 kişilik bir ekip ilk yönetmeliği hazırladı ve bu yönetmeliğin sekretaryası tarafımdan yapıldı. Orijinal tutanaklar halen bendedir. Yönetmelik bakan onayına giderken, bir akşam üstü saat 17.30 da rahmetli müsteşar Prof. Dr. İlhan Özdemir beni çağırarak yönetmeliğe tekrar göz atmamı istedi. Yönetmeliği incelediğimizde zamanın İEGM tarafından kafasına göre bazı değişiklikler yaptığını tesbit ettik ve yapılan değişiklikleri alel-acele tekrar eski haline koyarak bakan onayına çıkarttık. Bu arada yönetmelikte tabii ki bazı tutarsızlıklarda oldu. Yönetmeliğin çıkışı ile Türkiyede başta klinik farmakoloji derneği olmak üzere birçok toplantı, simpozyum ve panel yapıldı. İlk defa Good Clinil Practice doktorlara anlatıldı. Çeşitli toplantılarda tarafımdan anlatılan konulardan birisin başlığı ‘SAHTEKAR ARAŞTIRMACILARIN ERKEN TANISI’ idi. Halen onlarla uğraşıyoruz…
